Pro-State® Baseline and Progress Surveys

To help you track your journey toward better pelvic health, we’ve developed the Pro-State® Baseline and Progress Surveys. These evaluations are adapted from the clinically recognized NIH Chronic Prostatitis Symptom Index (CPSI) and consist of nine essential questions regarding your urogenital health. Each survey takes approximately 5 minutes to complete. At High Island Health, your privacy is paramount: your responses are used strictly for internal research and will never be shared with third parties.

Users who fill out the Progress survey after having used the Pro-State for two months are eligible to receive a free bottle of Essential water-based lubricant.
